About the Role
Key Responsibilities:
Service Coordination:
Match caregivers with clients based on individual needs and preferences.
Schedule and assign home care aides to ensure timely and efficient service delivery.
Client and Caregiver Support:
Serve as the primary point of contact for clients, families, and caregivers, addressing inquiries, resolving issues, and providing updates.
Monitor the quality of care and ensure client satisfaction by conducting periodic follow-ups.
HHA Exchange Software Management:
Utilize HHA Exchange software to manage client schedules, track caregiver assignments, and ensure compliance with service plans.
Input and maintain accurate client and caregiver records, ensuring data integrity.
Compliance and Documentation:
Ensure adherence to company policies, regulatory requirements, and client care plans.
Maintain comprehensive and up-to-date documentation of client interactions, schedules, and service plans.
Team Collaboration:
Work closely with the care management team to ensure seamless communication and coordination of services.
Assist with recruitment and onboarding of caregivers as needed.
Requirements
Experience:
Minimum of 1 year of experience in a home care setting, with a strong understanding of service coordination and client care.
Technical Skills:
Proficiency in using HHA Exchange software is required.
Education:
High school diploma or equivalent required; associate or bachelor’s degree in a healthcare-related field is a plus.
